1.         Copy of the I-797, Notice of Action and/ or the e-Filing confirmation receipt notice previously issued for previously filed I-90 application (the card was never received);

2.         Biometrics appointment notice;

3.         Original Green Card

4.         The mutilated Green Card (if the card was mutilated or partially destroyed );

5.         Error information statement (if the card contains erroneous data)

6.         Evidence of commuter status (to replace because of taking up commuter status);

7.         Driving license;

8.         Valid unexpired passport;

9.         Police report of stolen or lost green card (if the card was lost /stolen);

10.     Social Security Card;

11.     ID card;

12.     Birth Certificate;

13.     Evidence of last departure from the USA (airline ticket or boarding pass);

14.     Original court order or a certified copy of your marriage certificate reflecting the new name(to replace a card because of a name change);

15.     Copies of documentation to prove that the new data is correct (to replace a card because of a change of any other biographic data);

16.     Notice of residence authorization (if the card was lost, stolen or destroyed);

17.     Other document containing your name, date of birth, photograph and signature.

Note: Do not submit initial evidence and supporting documents when submitting the application. You must submit all required initial evidence, including your prior permanent resident card or other evidence of identity, and any supporting documentation when you appear in person at your local ASC.
