1.         Completed "Application for Immigrant Visa and Alien Registration" (Form DS-230 Part II). Do NOT sign the form until your interview with a consular officer.

2.         An Affidavit of Support (Form I-864) with sponsors' federal income tax return that was current if the affidavit of support has not been submitted to the National Visa Center.

3.         Passport: Each visa applicant must have a passport valid for at least six (6) months from the time of the visa issuance, and copies of the biographic pages; AND any previous travel documents with expired and/or current U.S. visas in them

4.         Birth Certificate and copies----English translation is required if written in another language

5.         Visa Photographs: Two (2) colored photographs printed according to specifications, provided in the visa application packet.

6.         Police Certificates: A police certificate is required from a country in which the applicant lived for more than one year after turning 16 years of age.

7.         Certificates of trial and court transcripts if you were ever convicted of a crime

8.         Medical examinations

10.     Marriage Records (if applicable):

If the marriage ended by divorce, annulment, disappearance or death of the spouse, legal and/or civil documentation must be presented attesting to the termination of the marriage (annulment decree, death certificate, a foreign divorce decree, or foreign death certificate).

11.     Military Record: Applicants who have served in the military should present a certified copy of their military records.

For U.S Citizens,

1)         Your birth certificate, if you were born in the United Stated;

2)         Certificate of naturalization or citizenship issued by USCIS or former INS;

3)         Form FS-240, Report of Birth Abroad of a Citizen of the United States, issued by a U.S. Embassy or Consulate;

4)         Unexpired U.S. passport; or

5)         An original statement from a U.S. consular officer verifying that you are a U.S. citizen with a valid passport.

For Lawful permanent residents,

1)         Both sides of Form I-551, Permanent Resident Card;

2)         An unexpired temporary I-551 stamp in either a foreign passport or DHS Form I-94, Arrival/Departure Document.

3)         If you have not yet received your card, submit your passport biographic page and the page showing admission as a permanent resident, or other evidence of permanent resident status issued by USCIS or the former INS.

Supporting Evidence of Husband-Wife Relationship:

1)         Your marriage certificate

2)         Wedding ceremony or wedding pictures

3)         If either you or your spouse were previously married, submit documents showing that all prior marriages were legally terminated, such as divorce decree

4)         Documentation showing joint ownership or property

5)         A lease showing joint tenancy of a common residence

6)         Joint credit card statement

7)         Joint bank statement

8)         Utility bills

9)         Airline tickets or boarding passes you traveling together

10)     Photos taken together

11)     Birth certificate(s) of child(ren) born to you, the petitioner, and your spouse together

12)     Evidence of correspondence to prove your relationship, such as letters, emails, remittance slips, chat history.

13)     Affidavits sworn to or affirmed by third parties having personal knowledge of the bona fides of the marital relationship.

14)     Letters from the employers of both parties, describing the employee's detailed marriage status and the beneficiary of the company's interests

15)     Any other relevant documentation to establish that there is an ongoing marital union.

13.     I-864 Sponsor/Petitioner¡¯s Supporting Documents

1)         Form I-864, affidavit of support

2)         Copy of unexpired US passport

3)         Copy of naturalization certificate/citizenship certification

4)         Copy of birth certificate (if you were born in the US)

5)         Copy of federal income tax return, including W-2 for the most recent tax year

8)         Bank statement showing present balance

9)         Documents of ownership of property of real estate

10)     Certificate of currently in the military armed service(if sponsor is on active duty in the U.S. Armed Forces)
